Supports Coordination

Supports Coordination (SC) is a service provided to people with developmental disabilities who typically need more supports than clinical services. The supports coordinator assists consumers with arranging and organizing community resources and natural supports to meet consumer outcomes. The frequency of contacts are based on consumer needs and occur in the home or in the community. Support coordinators are available 24 hours per day for crisis management.
