CSMClient Services Management (CSM)

CSM provides clinical case management services to adult consumers with severe and persistent mental illness. Case Managers provide home visits and community contacts that assist consumers with medication monitoring, education on mental illness, linking to community resources, vocation and recreational activity development, and supports the consumer to be as independent as possible within their community.

After hours coverage includes crisis intervention and, if needed, medication monitoring to prevent hospitalization. If you are receiving CSM services and have an after hours emergency, you may contact the case manager on-call 24 hours per day.
